Behavior Contracts

I really like the idea of doing behavior contracts with middle school students. I believe this technique is a little bit more difficult to do in a high school setting, but could be very beneficial in middle school settings.
If students misbehave or disobey class guidelines, they will be required to fill out a behavior plan. This behavior plan has three statements the student must complete: today I had a problem with, next time I will, the consequences for my actions should be. After the student completes the behavior form, they will sign and date it. A copy will be sent home to the parent for a signature and a second copy will be place in my behavior record.
If a student has multiple (3) incidents of the same issue, a phone call to the parent will be made and further consequences will be planned

Homework Policy

Homework is assigned on a regular basis. I strongly believe that it is imperative for students to continue their learning outside the classroom.
To keep track of who did not complete and/or turn in their homework on a given day, students need to fill out a pink slip and hand it in in place of their work. This is a better method for keeping track of who owes what and when; as well as limiting the amount of excuses that the teacher hears on a daily basis. Once a pink slip is received, appropriate next steps will be determined, from no-credit to an extension on the assignment. Most cases warrant for a warning with the first pink slip.

Test Policy

For students receiving a 75% or less on unit tests, I offer the opportunity for students to retest. Everyone has bad days and I want to be able to offer students the opportunity to make up for those bad days.
Students, in order to retest, have to fill out the form to the left. This form shows me that the student has taken the responsibility to improve their study skills and that they deserve to retest. This form also requires a parent signature.
